It was nice to find this thread and realize I&#039;m not alone. I&#039;ve contacted the Baltimore number the Italian promoter gave me about a hundred times, just get a general automated voicemail.

I even wrote a letter to the department requesting the forms. Nothin.

This is bullshit. Someone has the answer to this question. I&#039;m starting to believe at the end of rainbows there&#039;s no pot of gold, theres the TAJA1 (S5GA1) form. 
Well I live in Arizona, and it&#039;s not raining anytime soon.
